We’re taught to trust the system. When we walk into a hospital, we believe we’re in safe hands. That’s the story we tell ourselves.
But for nearly half a century, a parallel literature has been documenting a darker truth: medical care itself is a leading cause of death in America.
It began in 1974, when the Journal of the American Medical Association warned of tens of thousands of annual drug-induced deaths. It continued through the Institute of Medicine’s landmark 1999 report, To Err Is Human, which found that medical errors kill more people than car accidents, breast cancer, or AIDS. It culminated in a 2016 British Medical Journal study concluding that medical error is the third leading cause of death—behind only heart disease and cancer.
The estimates vary—from 44,000 to over 400,000 deaths annually—but the conclusion is consistent across all of them: preventable harm in healthcare settings is a public health crisis.
So why isn’t this on the CDC’s official ranking? Because death certificates aren’t designed to capture iatrogenic harm. They record the immediate biological event—cardiac arrest, sepsis, respiratory failure—but not the medication error, the hospital-acquired infection, or the unnecessary surgery that caused it.
The system is blind to its own failures. And that blindness costs lives.
